Job Overview The Nurse Practitioner Palliative Care will provide comprehensive palliative care services to clients, ensuring high-quality and professional care. The NP will conduct patient visits for new and established palliative care patients, coordinating their care plans within the designated team. These visits will occur in various settings, including patients' homes, facilities, and hospitals. The NP will also make independent decisions regarding patients' healthcare needs and deliver palliative care services as required. The Medical Director will be available for consultation, collaboration, and referrals as needed.

Requirements

  • Graduation from an accredited Baccalaureate school of nursing.
  • Completed training in the diagnosis and management of common as well as some complex medical conditions.
  • Current licensure in the State of California.
  • Certification as an Adult/Family/Pediatric/Women’s Health Nurse Practitioner.
  • Previous experience with computer-based systems is required.
  • A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Nice To Haves

  • Electronic Medical records is preferred.
  • Previous experience in hospice/palliative care, high-risk populations, including home-based care, preferred.
  • Bilingual in Spanish and English preferred.

Responsibilities
