As a Palliative Care Nurse Practitioner, you will play a vital role in supporting patients living with serious or life‑limiting illnesses by focusing on comfort, dignity, and quality of life. In this rewarding role, you will provide expert symptom and pain management while guiding patients and families through some of their most important healthcare decisions. You will lead thoughtful, empathetic goals‑of‑care conversations, ensuring patients and loved ones understand diagnoses, prognoses, and the full spectrum of treatment options. You will serve as a trusted advocate and clinical leader, coordinating care aligned with patient values, assisting with advance care planning, updating resuscitation status orders, supporting transitions, and evaluating patients for hospice eligibility. Requirements

  • Master's Degree in Nursing Required
  • 3 years experience in supervision and independent decision making in major nursing area preferred
  • Nurse Practitioners Certification (NP-C) required
  • BCLS - Basic Life Support required
  • Department Specific Certification preferred

Responsibilities

  • Providing specialized medical care for patients living with a serious illness or injury focused on providing relief from the physical & pain symptoms that come from the serious illness or injury, or side effects of treatment.
  • Hold goals of care meetings with patients and family to ensure patient and family understanding of their diagnosis and prognosis, available treatment options, benefits and burdens of treatment options and coordinating care for patients and their families based on their goals of care.
  • Change resuscitation status orders, withdraw of life support and evaluate for hospice.
  • Collects data in a systematic and ongoing process
  • Issues are identified and diagnoses are developed based on assessment data.
  • Prioritizes data collection based on the patient's condition and needs.
  • Identifies expected outcomes and plan based on diagnoses or issues.
  • Documents and submits required information and data in a timely fashion.
  • Clearly and accurately documents designated processes, policies, products, service offerings, etc.
  • Ensures that documentation is tailored to expected readers / users.
  • Uses correct terminology.
  • Conforms to required style and format.
  • Develops expected outcomes that provide direction for continuity of care.
  • Involves patient/family and other disciplines to determine expected outcomes.
  • Modifies expected outcomes based on changes in the assessment of the patient.
  • Includes the patient and others involved in the care in the evaluation process.
  • Evaluates the plan in relation to patient responses and expected outcomes.
  • Documents the results of the evaluation.
  • Directs the coordination of care across setting and among caregivers.
  • Engages in teamwork as a team player and a team builder.
  • Serves in key roles at work through shared decision making.
  • Defines a clear vision including goals and plans and measures progress.
  • Promotes nursing through participation in professional organizations.
  • Uses the results of quality improvement activities in nursing practice.
  • Uses creativity and innovation in nursing practice to improve care delivery.
  • Incorporates evidence based knowledge to initiate change in nursing practice.
  • Participates in quality improvement activities.
  • Adheres to the hospital and departmental attendance and punctuality guidelines.
  • Performs all job responsibilities in alignment with the core values, mission and vision of the organization.
  • Performs other duties as required and completes all job functions as per departmental policies and procedures.
  • Maintains current knowledge in present areas of responsibility (i.e., self education, attends ongoing educational programs).
  • Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time.
  • Demonstrates competency at all levels in providing care to all patients based on age, sex, weight, and demonstrated needs.
  • For non-clinical areas, has attended training and demonstrates usage of age- specific customer service skills.
  • Wears protective clothing and equipment as appropriate.
