Beyond direct treatment, wound care nurse practitioners help reduce hospitalization, improve quality of life, and support faster healing through coordinated, interdisciplinary care. They also educate staff and caregivers on prevention strategies, proper dressing techniques, and skin integrity monitoring, creating a culture of proactive care. Their presence in nursing homes leads to better outcomes, lower healthcare costs, and safer, more dignified care for residents. Details include: Perform comprehensive wound assessments on admission and routinely thereafter Diagnose and manage pressure injuries, venous/arterial ulcers, diabetic wounds, surgical wounds, and any other skin concerns Order and interpret relevant diagnostics (labs, imaging, cultures) Develop, implement, and adjust evidence-based wound treatment plans Select and prescribe appropriate wound care products, dressings, and therapies Perform or oversee debridement (sharp, enzymatic, autolytic) as clinically indicated Manage infection prevention and treatment, including antimicrobial stewardship Address pain management related to wounds and procedures Monitor healing progress and revise care plans to prevent complications Identify high-risk residents and implement early interventions Ensure appropriate nutritional support in collaboration with dietitians Collaborate and coordinate with physicians, nursing staff, physical/occupational therapy, dietitians, social services, vascular surgery, podiatry, dermatology, or infectious disease as needed Educate nursing staff on wound identification, staging, dressing application, and prevention Support caregivers and families with education on wound healing expectations Maintain accurate, timely, and compliant wound documentation Support CMS, state, and accreditation requirements related to skin integrity Reduce avoidable hospitalizations related to wound complications Improve healing rates and resident comfort Promote dignity, safety, and quality of life for residents Cross training to become a certified foot specialist as needed Travel to support locations as needed
