About The Position

Horizon Health in Elk Point, SD is seeking a Full Time Nurse Practitioner or Physician Assistant to provide direct patient care for all patients. Requirements

Responsibilities

Provides primary health care to patients, according to established evidence-based guidelines of clinical practice.
Ability to lead care teams to provide high quality healthcare in accordance with Horizon Quality Improvement Plan and meet established goals and benchmarks.
Management of long-term care for acute and chronically ill patients, including care coordination and development of care plans and self-management plans.
