Relocation Assistance (based on eligibility)  Duke Health is seeking a full-time APP, with a focus on behavioral health to join its established community-based faculty practice, Growing Child Pediatrics, based in Wake Forest, North Carolina.   Growing Child Pediatrics is one of the largest pediatric practices in North Carolina, with seven locations and 35 physician and advanced practice providers.  Founded in 1997, Growing Child offers fully integrated physical, mental and behavioral health care, including a cutting-edge treatment of child mental health and ADHD.  The practice provides care from newborns through college including well/sick care, after hours/urgent care, immunizations, sports and school physicals, minor surgical procedures, full laboratory services, health education and mental/behavioral health counseling.  The practice’s physicians joined the Duke Private Diagnostic Clinics in 2017.  APPs are eligible for the comprehensive benefits package through the Duke Health Integrated Practice.   Practice Highlights: Growing Child Pediatrics has six locations in Wake, Johnston and Durham Counties Patient Centered Medical Home, with a care team that includes MD, FNPs, PNPs and a LCSW.   Our highly desired Raleigh-Durham-Chapel Hill, NC area is consistently ranked as one of the top places to live.  With its array of family-friendly activities, exceptional schools and universities, and Research Triangle Park’s high-tech research and development center, Durham-Chapel Hill has something for everyone!  The area is home to Duke University, University of North Carolina, NC State University and several other renowned colleges.  To learn more about the Duke and Greater Triangle communities, visit: http://community.duke.edu/ and https://www.raleighnc.gov/

Requirements

  • Nurse Practitioners: Work requires completion of an accredited Nurse Practitioner program, with a Master's degree in Nursing.
  • PA: Graduate of ARC accredited Physician Assistant Program, preferably Master’s or Doctoral Degree.
  • BLS required.
  • NP: Current unrestricted RN license issued by or recognized by the NCBON with authorization to practice as an NP.
  • PA: Current unrestricted medical license issued by the NCMB with authorization to practice as a PA.

Nice To Haves

  • Prefer 1+ years of experience as an APP and/or RN in Pediatrics.

Responsibilities

  • Provide primary care for a panel of patients from birth to 21 years of age.
  • Coordinate the activities of the multidisciplinary primary health care team in an outpatient setting to ensure optimal patient care services, including but not limited to management of well patient care and follow-up, sick, urgent and newborn care including weight checks.
  • Supports patient self-management of disease and behavior modification interventions.
  • Participates in practice-based and division team meetings and quality improvement initiatives.
  • Coordinates continuity of patient care with external healthcare organizations and facilities, including hospital admission and discharge, ER visits and referrals from the primary care provider to a specialty care provider.
  • Conducts comprehensive, preventive patient screenings.
  • Assesses educational needs of patients and/or their families and provides education interventions as indicated and develops appropriate teaching tools as needed.
  • Utilizes appropriate resources within Duke to ensure all patient’s and/or their family’s education materials meet required standards.
  • Assists in teaching trainees rotating through on primary care pediatric rotations.
