About The Position
A large, mission-driven Federally Qualified Health Center (FQHC) is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join its outpatient primary care team. This is an excellent opportunity to practice comprehensive family medicine in a collaborative, well-supported environment focused on serving diverse and underserved communities. Position Highlights * 100% outpatient primary care * Adult and pediatric patient population * Team-based care model with physicians, APPs, nursing, behavioral health, and care coordinators * Mission-driven organization with strong leadership and clinical support * Predictable weekday schedule (no nights or weekends)
Requirements
- Licensed Nurse Practitioner or Physician Assistant
- Board certified or board eligible
- Active or eligible state license
- Primary care or family medicine experience preferred
- Commitment to community-based and underserved care
Responsibilities
- Provide high-quality outpatient primary care services to patients across the lifespan
- Diagnose, treat, and manage acute and chronic conditions
- Perform routine physical exams, preventive care, and health screenings
- Develop and implement treatment plans and patient education
- Document care in the electronic medical record (EMR)
- Collaborate with physicians and interdisciplinary care teams
- Participate in quality improvement initiatives and population health efforts
