We are looking for a dedicated, patient-focused Family Medicine Nurse Practitioner or Physician Assistant to provide high-quality care in our dynamic and collaborative practice. Requirements

Active Nurse Practitioner (NP) or Physician Assistant (PA) licensure in the State of Florida
Minimum of three years of clinical experience, preferably in family medicine or urgent care
Strong ability to build rapport and trust with patients
Demonstrated ability to manage a diverse patient population in a fast-paced clinical setting while maintaining high standards of care
Excellent written and verbal communication skills with team members and supervisors
Strong leadership qualities and teamwork-oriented mindset

Responsibilities

Diagnose, manage, and treat acute, episodic, and chronic conditions in a family medicine and urgent care setting
Prescribe appropriate medications and treatments following evidence-based practice guidelines
Perform comprehensive physical examinations and preventive screenings including Medicare Annual Wellness Visits
Telemedicine visits when medically appropriate
Develop and implement patient management policies and procedures
Coordinate referrals to specialists and medical facilities as needed
Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care
Engage in quality control measures (HEDIS)
Coordinate with Chronic Care Management Program team on eligible patients
Review and disposition of telephone and internal encounters
