We are looking for a dedicated, patient-focused Family Medicine Nurse Practitioner or Physician Assistant to provide high-quality care in our dynamic and collaborative practice. Requirements

  • Active Nurse Practitioner (NP) or Physician Assistant (PA) licensure in the State of Florida
  • Minimum of three years of clinical experience, preferably in family medicine or urgent care
  • Strong ability to build rapport and trust with patients
  • Demonstrated ability to manage a diverse patient population in a fast-paced clinical setting while maintaining high standards of care
  • Excellent written and verbal communication skills with team members and supervisors
  • Strong leadership qualities and teamwork-oriented mindset

Responsibilities

  • Diagnose, manage, and treat acute, episodic, and chronic conditions in a family medicine and urgent care setting
  • Prescribe appropriate medications and treatments following evidence-based practice guidelines
  • Perform comprehensive physical examinations and preventive screenings including Medicare Annual Wellness Visits
  • Telemedicine visits when medically appropriate
  • Develop and implement patient management policies and procedures
  • Coordinate referrals to specialists and medical facilities as needed
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care
  • Engage in quality control measures (HEDIS)
  • Coordinate with Chronic Care Management Program team on eligible patients
  • Review and disposition of telephone and internal encounters
