Summary We’re looking for a skilled and compassionate Nurse Practitioner or Physician Assistant to join our clinical team to provide weekly wound care rounding across assigned facilities. In this part-time role, you’ll focus on wound assessment, management, care planning, and collaboration with facility teams to improve patient outcomes.

Requirements

  • Active Nurse Practitioner or Physician Assistant license in the state of practice.
  • Master’s or higher degree in Nursing (NP) or Physician Assistant studies.
  • Strong clinical assessment skills with experience in wound management preferred.
  • Excellent communication and collaboration skills.
  • Ability to travel between facilities as required.

Nice To Haves

  • Wound care certification (WCC, CWON, CWS) preferred (or willingness to obtain).

Responsibilities

  • Conduct weekly wound care rounds at one or more assigned facilities, including thorough assessment of chronic, surgical, and pressure wounds.
  • Perform advanced wound care procedures such as wound assessment, dressing changes, debridement as appropriate, and wound-vac management.
  • Develop and update individualized wound care treatment plans, including ordering necessary diagnostics, topical therapies, and adjunctive treatments.
  • Educate patients, families, and facility staff on wound care best practices and prevention strategies.
  • Maintain accurate and timely documentation of assessments, care plans, interventions, and outcomes within the electronic medical record.
  • Collaborate with interdisciplinary partners (MD/DO, nursing, therapy, dietitians) to optimize wound healing and overall patient care.
  • Participate in quality improvement activities related to wound care clinical outcomes and processes.
