About The Position

This role is part of our Enterprise Wellness Program and focuses on providing virtual care for patients experiencing Alzheimer's disease, dementia, and other cognitive impairments. The Advanced Practice Provider (APP) will manage and support patients in a fully remote setting, delivering compassionate, specialized care tailored to cognitive health needs. We strongly prefer candidates with 3-5 years prior experience in Family Medicine Neurology or Cognitive Disorders, as this background will be essential in delivering high-quality, patient-centered care in a virtual environment. Schedule: Monday-Friday 8:00am-5:00pm 34 patient contact hours for 1.0 (6 hours admin weekly.) APP will participate in on-call rotation as well (1 in every 28 weeks at this time).

Requirements

  • Nurse Practitioner Master's Degree or Doctorate of Nursing Practice required.
  • License to practice as a Registered Nurse required.
  • Certification in the area of practice from a nationally recognized certifying body required.
  • ACNP, AGACNP, AGNP, PNP, or PNP-AC board certification preferred based on population served.
  • 3-5 years prior experience in Family Medicine Neurology or Cognitive Disorders preferred.

Responsibilities

  • Performs initial and periodic medical history, physical examinations, and daily assessments.
  • Records findings of examinations, assessments and plan of care.
  • Performs minor office-based medical and/or surgical procedures.
  • Writes appropriate prescriptions and administers medications.
  • Orders and interprets daily laboratory and radiology examinations.
  • Determines treatment plan for patient.
  • Documents all patient encounters, records observations, interventions, and actions in a medically appropriate manner on the patient's medical record.
  • Acts as a patient advocate for the population served.
  • Attends, participates and contributes to group/practice meetings and development of policies and procedures when requested.
