About The Position

Sutter Medical Group is seeking a dedicated and compassionate Skilled Nursing Facility Advanced Practice Clinician (APC) — Nurse Practitioner (NP) or Physician Assistant (PA) — to join our post-acute care team. This role focuses on delivering high-quality, patient-centered care to individuals residing in skilled nursing and rehabilitation facilities. Collaborate closely with facility staff, nursing teams, rehabilitation therapists, social workers, and case managers to deliver coordinated, patient-centered care.
