Requirements

  • Licensed to practice in the Commonwealth of Pennsylvania.
  • Board Certification/eligibility by American Nurses Credentialing Center.
  • English proficiency required.

Responsibilities

  • Performs examinations and formulates treatment of patients including appropriate history, physical examinations laboratory and other diagnostic testing and referrals as necessary.
  • Counsels and educates patients regarding their physical or mental health or delegates such education to a competent source.
  • Recognizes complicated medical conditions that require referrals.
  • Manages chronic conditions such as diabetes and high blood pressure.
  • Prescribes medications and other treatments.
  • Advises patients on how to prevent disease.
  • Refers patients to community resources and agencies.
  • Consults with their Collaborating Physician(s) as necessary.
  • Fully documents plan of care; and executes care in a timely manner.
  • Ensure timely completion of forms presented to them on behalf of the patient regarding the patient's physical or mental health conditions; for the proper billing of patients and third-party payers; and forms necessary for other funding sources.
  • Performs other work-related duties as assigned.
