About The Position

This job provides primary, or acute and/or behavioral healthcare services for certain acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization. This position involves a rotation of shifts, including weekends and holidays. daylight hours with weekend coverage required on a rotating basis.

Requirements

  • MSN from the practicing State Board of Nursing Approved Master’s Program (for Nurse Practitioner)
  • National Board Certification for Nurse Practitioners through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC) (for Nurse Practitioner)
  • Current State Certified Registered Nurse Practitioner License (for Nurse Practitioner)
  • Current State of PA RN licensure OR Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC) (for Nurse Practitioner)
  • Prescriptive Authority from the practicing state within 60 days of hire (for Nurse Practitioner)
  • Drug Enforcement Agency Licensure within 60 days of hire (for Nurse Practitioner)
  • Master’s degree in Physician Assistant Studies (for Physician Assistant)
  • Current PA Physician Assistant License (for Physician Assistant)
  • PA-C or board eligible (for Physician Assistant)
  • Ongoing successful recertification (NCCPA) as a PA-C (for Physician Assistant)
  • Drug Enforcement Agency Licensure Within 90 days of hire (for Physician Assistant)
  • CPR – American Heart Association
  • Act 34 Criminal Background Clearance Certificate
  • Act 33 Child Abuse Clearance Certificate
  • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
  • Four to ten years related experience and/or training; or equivalent combination of education and experience.

  • For Behavioral Health areas: Board Certified Psychiatric and Mental Health Nurse Practitioner (PMHNP-BC) by American Nurses Credentialing Center (ANCC), obtained within 2 years of hire.

Responsibilities

  • Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, discharge planning and providing accurate medical record documentation.
  • Communicates and coordinates patient care with patient, family, community health services and interdisciplinary team. Involves attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the CRNP's knowledge base or scope of practice.
  • Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds and provides wound care.
  • Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions and recommends community resources as appropriate.
  • Triages patient telephone calls, provides monitoring and continuity of care.
  • Participates, develops, and implements performance and/or quality improvement initiatives. Ensures compliance with all regulatory agencies.
  • Serves as the clinical expert and acts as a clinical resource.
  • May perform other duties such as daily rounding of patients, manage outpatient concerns and any other duty specific to specialty.
