  • Perform comprehensive assessments, develop treatment plans, and provide ongoing care management for residents, focusing on preventive care and chronic disease management.
  • Conduct regular rounds to assess residents’ conditions, adjust treatment plans, and address any emergent health concerns.
  • Collaborate with facility staff, family members, and other healthcare providers to ensure comprehensive, coordinated care tailored to the residents' needs.
  • Educate residents, families, and facility staff on disease prevention, wellness, and managing chronic illnesses to support better health outcomes.
  • Maintain accurate, timely, and thorough documentation within the electronic medical record (EMR) system, ensuring compliance with regulatory standards and organizational protocols.
  • Participate in quality improvement initiatives to enhance care delivery, reduce hospital readmissions, and optimize patient satisfaction within the facility.
  • Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field.
  • Masters Degree in Nursing or Physician Assistant Studies.
  • Strong leadership, interpersonal, and problem-solving skills preferred.
  • Must possess excellent written and oral communication skills.
  • Must possess traits of punctuality, organizational skills, and ability to function within a team.
