Requirements

• Masters-prepared graduate of an accredited school of nursing and Nurse Practitioner program for Nurse Practitioners.
• Current AZ RN and APRN license and certification as a FNP, ANP or ACNP for Nurse Practitioners.
• Graduate of accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the AZ State Board of Medical Practice for Physician Assistants.
• Active BLS certification from American Heart Association or American Red Cross at start date.
• DEA registration required.

Responsibilities

• Manage established patients with Parkinson's disease and other movement disorders and Alzheimer's disease and other cognitive syndromes.
• Ensure continuity and quality of longitudinal care.
• Learn Deep Brain Stimulation (DBS) device programming and management of new and emerging dementia therapies.
• Be part of a collaborative, patient-centered team and environment.
