About The Position

The position is for a full-time clinical role within the Family Medicine department at Advocate Health. The role requires 36 patient-facing hours per week, with a pay range of $50.05 to $75.10. The responsibilities include assessing, diagnosing, and managing treatment plans for patients, ordering and interpreting diagnostic studies, and providing health promotion and disease management counseling. The incumbent will work as part of an interdisciplinary team, maintain accurate documentation, and participate in quality and safety initiatives. The role also involves educating new team members and maintaining productivity and quality metrics to ensure optimal patient care delivery.

Requirements

  • Active RN and APRN/APRN-FPA/APNP license in the state of practice.
  • Active national board certification in area of clinical practice and populations served.
  • Active DEA registration prior to hire.
  • If practicing in Illinois, an active Illinois Controlled Substance License prior to hire.
  • Active BLS and/or ACLS, PALS, NRP as required by clinical practice prior to or within 6 months of hire.
  • Master’s Degree in Nursing.
  • No prior experience required.

Responsibilities

  • Assess, diagnose, and determine treatment and management plans appropriate for patients' age and clinical condition.
  • Order, perform, and interpret diagnostic studies and prescribe pharmacologic and non-pharmacologic interventions.
  • Manage conditions based on clinical indications, evidence-based care, and cost-effectiveness.
  • Provide health promotion, disease prevention, and disease management counseling to patients and families.
  • Collaborate with an interdisciplinary team and seek consultations or referrals as necessary.
  • Maintain accurate and timely documentation in the electronic medical record.
  • Participate in quality, safety, and peer review initiatives and departmental meetings.
  • Educate and onboard new team members and students.
  • Maintain standards of productivity and quality metrics for patient care delivery.
