About The Position

The CVCH Population Health Department leverages data-driven strategies to identify patient populations experiencing health disparities and provides both direct and indirect care aimed at improving health outcomes. The Population Health Nurse Practitioner’s or Physician Assistant Certified’s (Advanced Practice Provider-APP) plays a key role in supporting CVCH’s Medicare patient population by conducting comprehensive Annual Wellness Visits (AWV). The primary job function includes developing personalized care plans, review of patient’s medications, review of chronic conditions, assistance with advanced care planning, and screenings for mental health, substance use, fall risk, and social determinants of health. The Population Health Advanced Practice Provider promotes preventive care, patient engagement, and improves overall health outcomes by encouraging timely follow up with the patient’s primary care provider to address open care gaps and recommended next steps for ongoing health management. The Population Health Advanced Practice Provider is to be responsible for provision of comprehensive preventive family medicine services for the patients of Columbia Valley Community Health. Occasionally, the Nurse Practitioner or Physician Assistant Certified (Advanced Practice Provider-APP) may be requested to provide support to the family medicine practice, including the Express Care clinic (a type of urgent care setting). The position is eligible for additional compensation related to quality goals and has specific productivity goals that may differ from the regular family medicine providers.

Requirements

  • Education: Graduate from an accredited Nurse Practitioner Program or Graduate from an accredited Physician Assistant Program, Certification as a Physician Assistant.
  • Certification/Licensure: A valid license to practice as an ARNP with Evidence of Prescriptive Authority in the State of Washington OR PA-C in Washington State or eligible to obtain licensure. Basic Life Support (BLS) required. ACLS or ATLS may be used to substituted for BLS. All Providers must be eligible for enrollment in Medicare and must not be currently opted out of the Medicare program
  • Experience: Three (3) years of experience working in a community health center or managed care environment preferred.
  • A high degree of organization and accuracy. Good eye/hand coordination and precision, manual dexterity and patience. Perform complex and comprehensive medical care on a variety of patients. Must demonstrate ability to work with basic computer skills and MS Office products, such as Word, Excel, and Outlook. Ability to work in a fast-paced and often high-stress environment as well as the ability to adapt quickly to workload changes. Ability to apply basic statistical and mathematical concepts to patient data, test results and other related information.
  • Attuned to the unique needs of the indigent and under-served populations with practice emphasis placed on rural medicine needs. Must demonstrate leadership and team building skills. Must have strong listening, written and oral communication skills, with ability to communicate clearly. Must have the ability to understand and respond effectively and with sensitivity to appropriate age spectrums. Establish and maintain effective working relationships with those contacted in the course of work. Work with various cultural and ethnic groups in a tactful and efficient manner. Ability to work at all site locations with diverse staff as needed.
  • Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination. The ability to distinguish letters and symbols, as well as the ability to utilize telephones, computer terminals, and copiers is required. Requires standing and walking for prolonged periods of time. Occasionally lifts and carries items. Ability to help transfer patients. Requires working under stress in emergency situations and irregular hours.
  • On a continuous basis, know and understand all aspects of the job; intermittently analyze work papers, reports and special projects; identify and interpret technical and numerical information; observe and problem solve operational and technical policies and procedures; explain medical procedures. Maintain unimpeachable skill and knowledge in the principles of sound medical practice. Must be able to communicate the base of knowledge necessary to mentor medical personnel. Must demonstrate an ability to work under stress. Must demonstrate knowledge of administrative and personnel practices. Ability to read, analyze, and interpret general medical and other professional journals and governmental regulations. Ability to write reports and concise, accurate patient chart notes. Ability to effectively present information and respond to questions from supervisors, patients, third party payers, vendors and the general public.
  • Essential sensory requirements include the ability to: read computer keyboard, monitor, and documents; prepare and analyze documents; read extensively; see, recognize, receive and convey detailed information orally, by telephone and in person; convey accurate and detailed instructions by speaking to others in person and by telephone.
  • Tasks routinely involve a potential for mucous membrane or skin contact exposure to blood, fluids or tissue. Use of personal protective equipment (PPE), when appropriate, is required

Responsibilities

  • Conduct comprehensive Annual Wellness Visits for Medicare patients in both clinical and telehealth settings, including medication reviews, preventive screenings, recommended follow-ups, and closure of identified care gaps.
  • Develop individualized patient care plans based on patient needs, risk factors, and social drivers of health.
  • Facilitate advance care planning discussions with patients and the necessary documents to accurately record their care preferences.
  • Screen for mental health conditions, substance use, fall risk, and other health-related concerns.
  • Educate patients in preventive care, chronic disease management, and available community resources.
  • Provides comprehensive and continuous preventive primary care services in a patient centered medical home, including sanctioned non-clinic sites such as nursing homes, patient home, school-based clinics, mobile clinics, and telehealth settings.
  • Provides consultation to other clinic providers either in person or by telephone.
  • Consults with clinic physicians, specialists, pharmacists, nurses, or other health care professionals as appropriate.
  • Assesses patient needs for specialty referrals, diagnostic and in-patient care and makes appropriate referrals for service. High focus on Care Management and Social Determinant of Health needs.
  • Participates in development and utilization of CVCH practice guidelines and procedures.
  • Participates in audits and meets standards of CVCH Quality Improvement and Risk Management programs.
  • Responsible for accurate, timely, and complete documentation in patient records. Responsible for extensive chart preparation to edit diagnosis and problem list to ensure appropriate and relevant medical history, including claims generated outside of CVCH’s system.
  • Responsible for accurate and timely completion of billing documents.
  • May performs minor outpatient procedures in accordance with CVCH privileges.
  • Provides patients and family members with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and need for follow up as appropriate during each patient visit. Verifies and documents that the patient understands diagnosis, treatment and follow up recommendations.
  • Provides clinical supervision and appropriate training of ancillary health care staff to assure quality of care for CVCH patients.
  • Responsible for maintenance of credentials essential for practice, to include licensure, certification, and CME.
  • Attends all required staff meetings, provider meetings, and participates in CVCH committees as directed by Population Health Manager, supervising Physician, or CMO.
  • Leads by example and sets a standard of excellence for all team members.
  • Develops and maintains a practice plan which is in accordance with State of Washington Physician Assistant licensure requirements, in cooperation with supervising Physician and CMO.
  • Participates in Physician oversight activities as required.
  • Performs other duties and tasks as assigned by supervisor.
