Requirements

• Advanced practice provider certification as a Nurse Practitioner or Physician Assistant.

Responsibilities

• Provide team-based care to manage a panel of 1,000 patients in collaboration with a physician.
• Conduct in-person, virtual, or telephonic encounters including annual comprehensive visits, transitional care, and same-day visits.
• Utilize clinical decision support and AI integration through Epic tools to enhance patient care.
• Build and maintain long-term relationships with patients and their families.
• Participate in quality incentives and value-based care initiatives.
