About The Position

This Nurse Practitioner is hospital and/or office based and is responsible for management of oncology patients in assigned care setting. This person is accountable for the clinical care performed. Is responsible for initiating and following through on the physician’s plan of care. May be responsible to coordinate care between the outpatient and inpatient setting. May be responsible to perform triage services in assigned office. Works collaboratively with physician(s) to implement plan of care for oncology patients. Works collaboratively with colleagues and office personnel to assure coordination of care. Supports the Patient-Centered Medical Home (PCMH) by working with patients, families, providers and other staff to promote timely access to needed care, provide daily continuity of care coordination, and coach patients, families and care givers to understand the patient care plan and self-care management responsibilities.

  • Adheres to all clinic policies and procedures.
  • Active Nurse Practitioner License in good standing in the State of Ohio.
  • Demonstrated understanding of Nurse Practitioner theory, practice and medical care, medical equipment and instruments, and OSHA regulations.
  • Excellent interpersonal and communication skills.
  • Ability to analyze and determine solutions in order to best serve the patients.
  • CPR certification required.

Responsibilities

  • Provides direction to staff in regards to patient care.
  • Consistently arrives at work on time and adheres to regular work schedule.
  • Works closely with assigned physician.
  • See outpatients in the office at their scheduled appointment times.
  • Sees same day appointment patients to evaluate urgent/emergent needs.
  • Strives to meet required average number of visits per day.
  • At times of low visit volumes, coordinates with physicians to offset their workload.
  • Sees Infusion Room patients as needed for evaluation of patient and/or nursing concerns.
  • Responds to urgent needs in the Infusion Room, i.e., reactions, patient distress.
  • Completes documentation of visit in EMR within 72 hours of visit.
  • Ensures all visits are billed appropriately.
  • Completes Peer-to-Peer sessions as needed.
  • Addresses NP/PA worklist in a timely manner and assists with physician worklists as needed.
  • Orders tests and prescriptions based on patient need.
  • Completes New Patient Teaches as needed.
  • Covers inpatient hospital rounding as needed and as credentialed.
  • Evaluates Social Determinants of Health and refers to community resources as appropriate.
  • Covers outpatient clinics in Fulton County, Henry County, Holland-Sylvania and Maumee, as needed.
  • Complete progress notes on each patient daily unless the attending oncologist/hematologist has already made a note.
  • The Nurse Practitioner rounds on the patient making sure he/she knows the patient’s clinical course and notify the attending oncologist/hematologist of any important changes.
  • Responds to phone calls from registered nurses regarding assigned patients.
  • Help with education and communication for patients and families as appropriate during hospitalization and during follow-up visits regarding disease process, chemotherapy, side effects, etc.
  • Dictate discharge summaries for the inpatient as directed.
  • Coordinates with office to ensure discharged patients are scheduled for a post-discharge follow-up appointment.
  • Coordinate care between inpatient and outpatient office settings.
  • May share weekend and holiday call with oncologist/hematologist.
  • Recruit patients for research protocols, as applicable.
  • Completes assigned clinical projects for review and approval by physician.
  • Responsible for implementation of approved projects.
  • Responsible for evaluation of these projects as assigned.
  • Assumes responsibility for projects which are of importance to the Practice. Examples include:
  • Survivorship program
  • Smoking Cessation
  • Palliative Care
  • Oral Chemotherapy
  • Other as specified by the practice
  • Manages office prescription line and responds to patient refill requests.
  • Documents in the EMR.
  • Completes telephone assessment of patients calling to report a clinical problem.
  • Completes appropriate documentation in EMR.
  • Documents recommendations and actions to be taken by patient.
  • Sees patients requiring urgent evaluation and develops action plan based on assessment findings.
