As a Nurse Practitioner at Greater Good Health, you'll be at the heart of our mission to provide exceptional care to our I-SNP population. Our High Risk program is designed for patients with significant health needs and complex conditions to prevent acute care readmission and over-utilization of inpatient care. A GGH High Risk Nurse Practitioner will provide in-facility, in-home, and virtual clinical support for these patients as well as patients returning home from an acute inpatient admission. The goal is to prevent readmission, but also to provide support for clinical and psychosocial factors that contribute to the cyclical utilization of healthcare. We want our clinicians to take the time needed to address the patient's needs and concerns.
