We are looking for a compassionate Nurse Practitioner to join our hospice team. Requirements

• Minimum 5 years of nursing experience (preferred)
• Current NP or CNS certification in the state of Indiana
• Valid driver’s license, auto insurance, and reliable transportation

Responsibilities

• Conduct required face-to-face hospice visits and report findings to the Hospice Medical Director
• Develop, coordinate, and document patient plans of care
• Collaborate with attending physicians and IDG members
• Prescribe medications (including controlled substances) within scope and delegation
• Order diagnostics and treatments as appropriate
• Educate patients, families, staff, and community on hospice and palliative care
• Participate in IDG and QAPI meetings
• Maintain accurate clinical documentation for compliance and reimbursement
• Supervise and evaluate home health aides as appropriate
• Complete required training modules
