As a core member of the Stafford Services clinical team, you will be the front-line leader driving optimal outcomes and quality improvement. We provide Advanced Practice Nurses (APNs) and Podiatric services to skilled nursing, long term care and assisted living facilities. Our multi-specialty clinical care services help you achieve optimal outcomes and reduce hospital readmissions. Requirements
- Active and unrestricted Nurse Practitioner License in the State of Ohio.
- Active Nurse Practitioner Certification.
Nice To Haves
- Skilled Nursing/Rehab experience is preferred.
- Will consider highly motivated new graduates, though 3+ years of prior RN experience is preferred for this specialized setting.
Responsibilities
- Provide expert routine and acute medical care directly to SNF residents.
- Conduct thorough assessments, manage complex chronic conditions, and oversee personalized care plans.
- Partner seamlessly with facility nursing leadership and attending physicians to ensure integrated care.
- Actively support quality improvement initiatives, focusing specifically on reducing preventable hospital readmissions
