Requirements

• Must be a Board Certified Nurse Practitioner.

Responsibilities

• Serve as the primary care provider to residents in a skilled nursing facility.
• Conduct physical examinations and assess patients' health by visiting them.
• Collaborate with medical attending/physician and nursing home staff to provide optimal care.
• Build relationships with residents and families, educating them and formulating evidence-based care plans.
• Ensure compliance with federal, state, and local legal and professional requirements.
• Maintain professional knowledge by attending workshops and participating in professional societies.
• Ensure a safe and clean working environment by adhering to procedures and regulations.
• Review and ensure timely and efficient charting, documentation, and administration of care.
