We want our clinicians to take the time needed to truly address the patient's needs, focusing on patient care rather than volume.

Requirements

• Licensed as a Nurse Practitioner in the State where HarmonyCares Medical Group office is located.
• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.

Responsibilities

• Provide primary care to high acuity patients in their home Monday - Friday.
• Manage chronic conditions with the support of a Primary Care Medical Group.
• Work with an assistant to support appointments in select markets.
• Utilize a company car with a driver in select markets.
