Requirements
- Unrestricted Arizona Nurse Practitioner OR Physician Assistant license or ability to obtain prior to start date
- National Nurse Practitioner certification through AANC or AANP; OR Physician Assistant national certification through NCCPA
- Current DEA registration or ability to obtain prior to start date
- Current BLS certification or ability to obtain prior to start date
Nice To Haves
- Experience in providing patient care under a Value Based Care Model
- Experience in outpatient Primary Care
- Knowledge of HEDIS measures, CAHPS, HOS, HCC Coding documentation
- Proficient in Medicare Annual Wellness visits
- Proficient in EMR systems & technology (EPIC Preferred)
Responsibilities
- Practice outpatient Primary Care at the full scope of your license
- Chose either a four-day or five-day work week schedule
- 36 patient contact hours each week
- 18-22 patients per day on average
