As part of a start-up subsidiary of Maine Health, you will play a crucial role in shaping our clinical practices and patient experiences from the ground up. This position requires flexibility, a high level of autonomy, and a proactive approach to both problem-solving and patient care. The APP manages a comprehensive patient panel as a key member of the care team, fostering trust-based relationships and proactive healthcare management. They collaborate closely with other team members, including nurses, medical assistants, and specialists, to develop, update, and prioritize patient care plans. The APP ensures team-driven adherence to these plans, focusing on holistic health outcomes, patient satisfaction, and effective resource utilization, demonstrating a commitment to the collaborative care model.
