About The Position

Seeking an experienced Nurse Practitioner Leader/Navigator for the HMH Health and Wellness Center, our outpatient facility in Edison, NJ. This leadership role combines clinical team management with a key patient navigation function. The NP will manage all clinical and support staff, oversee daily operations, and serve as the primary point of contact for new patients, directing them to the appropriate specialty care for ortho, neuro, spine and PMR care. Responsibilities also include comprehensive patient assessments, management of complex health problems, and ensuring JCAHO compliance. A minimum of 5 years of leadership experience is required. This is a full-time, Monday-Friday (8:00 AM - 5:30 PM) position. This position will oversee patients needing assistance with a referral to ortho, neuro, spine and PMR providers.

Requirements

  • Master's Degree in Nursing (MSN).
  • Minimum of 5 years of RN/Specialty experience.
  • Minimum of 5 years of leadership or administrative experience.
  • Current NJ State Professional Registered Nurse (RN) License.
  • Current NJ Advanced Practice Nurse (APN) License.
  • Board Certification as an Adult Nurse Practitioner.
  • Active NJ CDS and DEA registrations for prescriptive authority.
  • AHA Basic Life Support (BLS) Certification.
  • Advanced Cardiac Life Support (ACLS) Certification.
  • Proficiency with Microsoft Office Suite (Word, Excel, PowerPoint).

Nice To Haves

  • Minimum of 2 years of experience in a Nurse Practitioner role.

Responsibilities

  • Provides administrative support and direction for the day-to-day operation of the assigned unit.
  • Develops short- and long-term unit goals aligned with organizational goals.
  • Manages time and resources effectively, handling multiple priorities with appropriate delegation.
  • Develops and implements activities that promote patient satisfaction.
  • Provides timely notification of critical events according to all regulatory and organizational policies.
  • Ensures all team members are knowledgeable in emergency preparedness and disaster response, developing a unit-specific plan and participating in all required drills.
  • Promotes the cost-effective use of organizational resources to support budget objectives.
  • Is responsible for financial analysis at the unit level, considering case mix, acuity, volume, and staffing.
  • Reviews and acts on variance analysis reports (including nursing hours per patient day, cost per unit of service, FTEs).
  • Implements, monitors, and evaluates action plans to correct variances.
  • Prepares and submits monthly variance and justification reports to the Director/CNO.
  • Maintains clinical expertise and credentials while seeking professional growth.
  • Ensures appropriate educational activities are planned and implemented for patients, families, and team members to promote evidence-based practice and meet standards of care.
  • Provides and supports professional growth opportunities for team members to develop and enhance their leadership skills.

  • Ensures the provision of competent care to patients and families, and assures nursing care is delivered and documented through the nursing process.
  • Participates in and implements recommendations from Best Care Inspections for regulatory readiness.
  • Obtains a thorough patient history, including chief complaint and analysis of symptomatology.
  • Performs an appropriate physical examination, recognizes normal and abnormal findings, and records them clearly.
  • Makes recommendations based on the data gathered for ordering studies, developing a problem list, formulating a diagnosis, and providing patient education.
  • Orders, interprets, and evaluates diagnostic and laboratory tests to assess patient problems and healthcare needs.
  • Writes orders to implement pharmacological and non-pharmacological therapeutic plans.
  • Communicates pertinent patient information (including clinical problems, test results, and treatment plan updates) with collaborating physicians.
  • Collaborates effectively with physicians, nurses, and other healthcare team members.
  • Maintains professional accountability to meet all job requirements, skills, and competencies.
  • Performs other duties and/or projects as assigned.
  • Adheres to HMH Organizational competencies and standards of behavior.
