ESSENTIAL DUTIES AND RESPONSIBILITIES To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. · Conduct in-home or facility-based visits to hospice patients for initial assessments, face-to face visits, routine follow-up visits, and symptom management. · Collaborate with the Medical Director or Associate Medical Director and the IDT to develop, implement, and revise individualized care plans. · Provide expert pain and symptom management consistent with hospice philosophy and guidelines. · Serve as a clinical resource to hospice nurses and other staff. · Participate in IDT as needed. · Educate patients and families on disease progression, end-of-life expectations and hospice treatment options. · Document all patient care accurately and timely in the electronic medical record. · Participate in on-call coverage as needed. · Comply with company policies, procedures, and guidelines.
