About The Position

Nurse Practitioner Hospice (Full-Time) Come join a team of seasoned clinicians, in a non-corporate environment, at Care Partners - one of Oregon's few 5-star quality, nonprofit hospices! For over 43 years, we have cared for hospice patients across the Portland Metropolitan area, bringing interdisciplinary and holistic hospice services to patients and their support systems. The Nurse Practitioner (NP) is a member of the hospice interdisciplinary team who works autonomously, but in collaboration (as necessary) with the hospice Medical Director, hospice physician(s), and members of the Interdisciplinary Team (IDT), to ensure appropriate, quality hospice services to patients and their families. This position may also provide physician-based consultative and administrative services, within accepted standards of advanced nursing practice and applicable laws and regulations.

Requirements

  • BSN from an accredited school of nursing, with a master's degree in nursing from an accredited Nurse Practitioner Program or a Doctor of Nursing Practice (DNP) Doctoral Degree.
  • Possess current and unencumbered license as a Nurse Practitioner in Oregon, with prescriptive privileges.
  • Possess insurance required by state law for Nurse Practitioner to practice (and prescribe) in Oregon or be eligible for such insurance requirements to be satisfied by group coverage with an employer.
  • Demonstrated understanding and acceptance of philosophy of hospice, hospice principles of care and the medical aspects of hospice care.
  • Demonstrated understanding of federal and state laws and regulations pertaining to hospice programs.
  • Demonstrated excellence with communication skills and ability to be professional and courteous.
  • Demonstrated computer literacy in Microsoft Office programs.
  • Ability to obtain proficiency in electronic medical record (EMR).
  • Demonstrated leadership capabilities with the ability to relate well to staff and other professionals.
  • Ability to work as a member of an interdisciplinary group.
  • Current BCLS certification.
  • Must have a valid driver's license, valid auto insurance, acceptable driving record and have access to a reliable automobile to travel to and from patient homes.
  • New employees are required to complete tuberculosis screening and provide proof of Hepatitis B vaccination or signed declination.

Responsibilities

  • Demonstrates knowledge of and appropriate application of Medicare CoP's eligibility requirements and medical necessity of hospice services (as outlined by CMS's LCDs) for documentation of patient's status for review by Medical Director and hospice physician(s).
  • Completes face-to-face encounters with Medicare-benefitted hospice patients who are entering their 3rd or subsequent benefit period.
  • Evaluates patient's physical condition as it relates to the terminal diagnosis to assess how the patient's condition correlates to eligibility requirements.
  • May perform patient visits as requested by the Medical Director, Hospice physician(s), and/or Clinical Manager to assess a patient's physical condition and to assist with the evaluation and treatment of the patient.
  • Serves as patient's Attending Physician, if the patient has no Primary Physician and selects the N.P. while a patient of Care Partners.
  • Administers medications and treatments as prescribed and in accordance with nursing standards of practice.
  • Provides education and interventions to support the effective management of the patient's pain and other physical symptoms as well as assesses the patient/family's response to intervention and treatment, as appropriate.
