About The Position

Hearth Hospice is currently seeking dedicated Hospice Nurse Practitioners to join our team on a PRN basis in the Fort Oglethorpe, GA area. As a Hospice Nurse Practitioner, you will play a crucial role in delivering exceptional patient care to individuals facing terminal illnesses. Your responsibilities will encompass evaluating and assessing health histories, promoting health and well-being, diagnosing medical conditions, and managing health problems through the development of personalized care plans. You will work closely with patients, their families, and other healthcare practitioners to ensure a holistic and supportive approach to care. In this role, you will be responsible for delivering compassionate medical care, ensuring compliance with the state Nurse Practice Act, and collaborating with the patient's attending physician and hospice medical director to assess eligibility for hospice care. You will coordinate and oversee patient care in collaboration with the interdisciplinary team, engage in patient and family education, and provide recommendations for certification and recertification for hospice care. Staying informed about current Medicare/Medicaid regulations and ensuring compliance will also be key aspects of your role.
