About The Position

Called to serve patients in their homes, the Nurse Practitioner approaches each day with a passionate commitment to delivering personalized, one-on-one care — helping patients regain their independence and achieve their health goals. In this role, you will provide hospice care through the evaluation and assessment of the overall medical conditions of patients, complete face to face assessments and evaluate hospice eligibility. Requirements

  • Licensed Nurse Practitioner with the State Nurse Practice Act Licenses in the stat of operations where assigned branch is located
  • Minimum of one year of Nurse Practitioner experience
  • Minimum of six months to one year of experience in hospice or pain and symptom management
  • Reliable transportation and valid and current driver’s license

Nice To Haves

  • Familiar and comfortable with technology. HCHB experience is a plus

Responsibilities

  • Provide Medical Care, palliates, and manage terminal illness
  • Complete History and Physical examinations by evaluating the patient’s medical condition and health history, ordering hand interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documenting findings
  • Develops individualized plan of care, prescribe medication and/or treatments, obtain consultations
  • Complete hospice face-to-face visit requirements
  • Reviews and develops protocols for treatment, and consults the collaborating physician when the patient’s plan of care is outside standardized practice and protocols
