We are seeking an experienced and compassionate Nurse Practitioner to join our Suffolk, VA team on a full-time basis. This position enables healthcare professionals to make a meaningful impact on patients’ lives in post-acute environments, supported by an inclusive and collaborative organization.

Requirements

• Completion of an accredited Nurse Practitioner program (NP degree required)
• Valid Virginia NP license and current DEA registration
• Competency with Electronic Medical Record systems
• Demonstrated compassion, professionalism, and ability to excel in a team environment
• Excellent communication, problem-solving, and organizational skills

Nice To Haves

• Post-acute care experience is preferred, but not required

Responsibilities

• Provide high-quality, evidence-based patient care within post-acute care settings
• Coordinate and collaborate with multidisciplinary teams to ensure the best patient outcomes
• Effectively utilize Electronic Medical Records (EMR) for accurate and efficient documentation
• Build professional, empathetic relationships with patients, their families, and fellow healthcare professionals
