About The Position

At Signify Health, part of CVS Health®, we are on a mission to build trusted relationships to make people healthier. We are hiring board-certified nurse practitioner contractors to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. This role involves conducting in-home visits that focus on a person’s overall health and well-being and identifying potential barriers to care. Our NP contractors can earn a competitive wage while managing other personal and clinical commitments, as they will be provided a pre-populated, patient-specific form for each evaluation. It is important to note that NPs do not prescribe medicine, order lab tests, or alter people’s current treatment regimen.

Requirements

• Board-certified nurse practitioner license required.
• State license required for practice.

Nice To Haves

• Experience in conducting in-home health evaluations.
• Strong communication skills to connect with patients one-on-one.

Responsibilities

• Conduct in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.
• Perform medication and medical history reviews and physical evaluations in health plan members' homes.
• Identify potential barriers to care during evaluations.
