About The Position
The Family Nurse Practitioner (FNP) will assess and provide care to a selected group of patients in collaboration with physicians at Montefiore Medical Center. This role involves carrying out the plan of care for the patient panel, contributing to the objectives of the division, and participating in performance improvement and research activities. The position is based at the Montefiore Bronx Health Collective, a community health center that provides comprehensive primary care services to underserved populations, including pediatrics, reproductive health, and adult medicine.
