The position is for a full-time role in Family Medicine at Advocate Health, specifically within the AMG Westhaven department. The role involves clinical responsibilities such as assessing, diagnosing, and managing treatment plans for patients of various ages and conditions. The candidate will work as part of an interdisciplinary team, providing health promotion, disease prevention, and management counseling. The position requires maintaining accurate documentation and facilitating coordinated care among healthcare team members. Additional responsibilities include participating in quality and safety initiatives, onboarding new team members, and maintaining productivity and quality metrics. The working hours are typically Monday through Friday, from 7:00 am to 5:00 pm, with specific hours to be determined upon hire.

  • Assess, diagnose, and determine treatment and management plans for patients.
  • Order, perform, and interpret diagnostic studies.
  • Manage conditions based on clinical indications and evidence-based care.
  • Provide health promotion, disease prevention, and management counseling.
  • Collaborate with an interdisciplinary team and seek consultations as needed.
  • Maintain accurate documentation in the electronic medical record.
  • Participate in quality, safety, and peer review initiatives.
  • Facilitate consistent care and communication among healthcare team members.
  • Perform office or hospital procedures as per specialty practice.
  • Participate in the education and onboarding of new team members.
  • Active RN and APRN/APRN-FPA/APNP license in state(s) of practice.
  • Active national board certification in area of clinical practice.
  • Active DEA registration prior to hire.
  • Active Illinois Controlled Substance License if practicing in Illinois.
  • Active BLS and/or ACLS, PALS, NRP as required by clinical practice.
  • Master’s Degree in Nursing.
  • No prior experience required.
  • Demonstrated high level of clinical proficiency.
  • Excellent decision-making skills.
  • Ability to work independently and as part of a healthcare team.
  • Proficiency with electronic medical records.
