Job Summary PURPOSE OF YOUR JOB DESCRIPTION: The purpose is to support the Family Medicine Physician Practice and to provide coverage when the providers are out of the office. DELEGATION OF AUTHORITY: They will report to the providers they work with and will utilize our nurses and MA’s in the offices as well. Requirements

  • Primary Care NP certified in Massachusetts
  • 3 years experience in a Primary Care setting
  • Proficient Computer Skills (MS Word)
  • EMR experience required
  • Ability to document in electronic health record completely and accurately
  • Knowledge of clinical reminders and how to use them appropriately
  • Good communication skills, both spoken and written
  • Willing and able to work in a team environment
  • Able to change with the ever evolving healthcare field

Nice To Haves

  • Master Degree
  • ACLS
  • PALS

Responsibilities

  • Diagnose and Treat acute and chronic Illness
  • Perform age-appropriate history and physical
  • Order and interpret diagnostic and therapeutic tests relative to patient’s age-specific needs
  • Prescribe appropriate pharmacologic and non-pharmacologic treatments to patient s age specific need
  • Monitor the effectiveness of treatments
  • Provide education of acute and chronic medical conditions
  • Managing Chronic Medical Conditions
  • Reconciling medication list
  • Facilitating appointments to specialist
  • Providing thorough exams and evaluations of patient’s health status
  • Facilitate appropriate chronic care management (i.e. Diabetic eye exams, foot exams and nutritional counseling)
  • Preventive Medicine
  • Keeping track of and referring patient to appropriate preventive medical screenings (e.g. Mammograms and Colonoscopies)
  • Counseling and reinforcing patients in healthy life style habits
