Care is delivered both digitally and at physical locations across Massachusetts, New Hampshire, and Maine. In addition, the group provides outpatient surgery and endoscopy, imaging, cardiac testing, and infusion services. There is a strong commitment to ensuring patients receive coordinated and comprehensive experience at all locations, guaranteeing that each patient has access to the appropriate level of care throughout the system’s delivery sites. We are seeking a full-time Nurse Practitioner (40 hours per week) to join our Family Medicine practice at 21 Elm Street in South Deerfield, MA. Our hours of operation are Monday–Friday, 8:00am–5:00pm, with a weekly schedule of 36 hours of direct patient care and 4 hours of administrative time, designated on Fridays. Our South Deerfield office is a close-knit, community-centered practice dedicated to comprehensive primary care for patients of all ages. We provide physical exams, routine screenings, preventive care, and the diagnosis and management of acute and chronic conditions—with a strong focus on prevention and long-term wellness. Primary care is about relationships, and we take pride in truly knowing our patients to deliver exceptional, personalized care. This role will support a patient panel of approximately 1,000 patients.
