The Mass General Brigham Medical Group is a system-led operating entity formed by Mass General Brigham to deliver high quality, low cost, innovative community-based ambulatory care. This work stems from Mass General Brigham’s unified system strategy to bring health care closer to patients while lowering total health care costs. The Medical Group provides a wide range of offerings, including primary care, specialty care, behavioral and mental health, and urgent care, both digitally as well as at physical locations in Massachusetts, New Hampshire, and Maine. The group also offers outpatient surgery and endoscopy, imaging, cardiac testing, and infusion. We share the commitment to delivering a coordinated and comprehensive experience across all locations, ensuring the appropriate level of care is available to every patient across our care delivery sites. Our Extended Care team consists of board-certified physicians, nurse practitioners, and physician assistants who provide episodic and chronic care to patients staying in several skilled nursing or long-term care facilities across the North Shore. Our dedicated care team closely monitors each patient's progress, coordinates and consults with specialists, and keeps both the patient's primary care physician and family informed. When it is time for a patient to return home, the team coordinates a plan of care with the patient’s primary care physician, ensuring there is a smooth transition back to daily life. The Nurse Practitioner (NP) is a licensed provider. The NP is responsible for assessing and managing various patient populations, including those with medical and/or surgical conditions, as well as providing diagnostic and therapeutic interventions, developing appropriate plans of care, and conducting ongoing evaluations. May also perform additional duties, such as precepting a small group of learners.

Requirements

  • Master's Degree Nursing required
  • Advanced Practice Registered Nurse [APRN-NP - State License]
  • 2 years of licensed nurse practitioner experience
  • NPs/PAs are required to see 12+ patients per day when assigned to work in the Skilled Nursing Facility and/or 6-8 patients per day when performing Home Care visits.
  • NPs/PAs assigned to work in the Skilled Nursing Facility must be available to respond to their pagers within 15-minutes between the hours of 8:00am and 5:00pm, Monday-Friday, and are also required to be on-call and available via phone one night per week (5:00pm to 8:00am) and two weekend days per month (8:00am to 8:00pm).

  • The ideal candidate will bring experience caring for medically complex adult patients in settings such as skilled nursing or long-term care facilities, geriatrics, hospital medicine, primary care, post-acute care, or home-based care models.
  • Experience coordinating care across disciplines and managing both acute and chronic conditions is strongly preferred

Responsibilities

  • Assisting individuals and families with their care needs by obtaining health and medical histories, performing physical examinations and assessing health problems
  • Caring for patients with acute and chronic life-limiting illnesses by managing therapeutic regimens, prescribing medications and performing other studies and activities that are deemed appropriate
  • Managing and evaluating new and follow-up patients with physician input obtained as necessary
  • Assuming all SNF/LTC care management responsibilities
  • Facilitating communication with the patient, the patient's family or significant other, and the referring physician
  • Working in collaboration with the patients' attending physicians at the SNF/LTC facility, as well as providing continued collaboration with the patients' primary care provider(s)
  • Provides direct care, counseling, and education to a designated patient population in ambulatory, inpatient, operative, and/or procedural settings.
  • Performs complete histories and physical examinations.
  • Orders, interprets, and evaluates appropriate laboratory and diagnostic tests.
  • Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
  • Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.
  • Consistently provides high-quality and timely documentation, including admission and progress notes, procedure notes, operative notes, and discharge summaries.
  • Performs bedside procedures as are appropriate to the patient population.
