About The Position

NYNJ Psychiatric Associates is a fast growing & vibrant Psychiatric group practice that provides services in assisted living & long-term care facilities throughout NY & NJ. Our well-respected team has been providing services for 14 years and currently has 35 Nurse Practitioners on the team. Our growth, to become one of the largest providers in the state of NJ, has largely been organic & word of mouth. Facilities refer us to other facilities, and our clinical partners will often do the same (pharmacy consultants, PCP's etc) - due to the quality of care and clinical support that we provide. Due to demand for our services, we are currently expanding in the state of NY. Position Overview We are seeking a compassionate and experienced Nurse Practitioner to provide high-quality psychiatric care to residents of nursing homes and assisted living facilities throughout New York State, with a primary focus on the five downstate boroughs. This opportunity focuses on geriatric and adult psychiatry, emphasizing collaborative, interdisciplinary care and clinical leadership. In addition to direct patient care, the Nurse Practitioner will help lead a clinical team including other Psychiatric Nurse Practitioners (PMHNPs), Psychologists, and LCSW’s, supporting clinical excellence, professional development, quality assurance, and regulatory compliance across long-term care settings. Some unique features of this role include the diversity of clinical conditions, varied work settings (non-monotonous routine), with flexible work hours within the business day, and with some weekly hybrid hours (some work from home/tele hours).

Requirements

  • Active, unrestricted New York State NP License
  • PMHNP needed based on state
  • DEA registration (or eligibility)
  • Experience or strong interest in geriatric psychiatry, long-term care, or consult-liaison psychiatry
  • Ability to collaborate effectively with interdisciplinary teams
  • Effective oral and written communication in English
  • Basic proficiency with technology, including electronic health records (EHR).
  • Strong organizational and documentation skills, with attention to regulatory compliance.
  • Ability to provide in-person services at assigned facilities, up to 45 minutes.
  • Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
  • Full Vaccination and Booster Status may be required in some facilities.

Responsibilities

  • Conduct initial psychiatric evaluations and ongoing medication management follow-up care for residents in long term and assisted living settings
  • Manage psychiatric conditions, mainly including mood disorders, dementia-related behaviors, anxiety disorders, and psychotic disorders
  • Provide follow-up care and adjust treatment plans based on patient response and facility needs
  • Support onboarding and clinical development of care team members, as appropriate
  • Participate in interdisciplinary treatment team meetings with nursing, social work, and facility leadership, as needed
  • Support quality assurance and best practices in psychiatric care delivery
  • Maintain timely, accurate, and compliant clinical documentation in the electronic health record
  • Participate in audits, care plan meetings, and regulatory reviews as required
  • Maintain compliance with CMS, NYS DOH, HIPAA, and facility-specific regulations
