The Nurse Practitioner works on problems of diverse scope and complexity ranging from moderate to substantial. Requirements

• A Bachelor's degree.
• 8 or more years of technical experience.
• 2 or more years of project leadership experience.
• Active, unrestricted Nurse Practitioner/PA license in the appropriate state.
• Passionate about contributing to an organization focused on continuously improving consumer experiences.

Nice To Haves

• Masters in Science of Nursing.
• Bilingual in English/Spanish, with the ability to speak, read, and write in both languages without limitations or assistance.

Responsibilities

• Work in a team-based care environment focused on quality of care for patients.
• Collaborate with a care team consisting of doctors, advanced practice professionals, Pharm D, Care Coach Nurse, MA, Behavioral health specialist, Quality based Coder, Referral Coordinator and more.
• Provide a concierge experience, multidisciplinary services, coordinated care supported by analytics and tools, and deep community relationships.
• See fewer patients and spend more time with them due to robust support.
• Conduct acute care visits, TCM's, and Home visits.
