Bristol Health is seeking a nurse practitioner to join a collaborative team of committed behavioral healthcare professionals to provide quality care and substance recovery services to individuals with a broad range of mental health illnesses in the greater Bristol community. This opportunity is at our outpatient Counseling Center of Bristol Health and is an exciting opportunity for a provider that thrives in a multidisciplinary team approach to treatment and enjoys an environment that promotes continuous learning and knowledge sharing. Since 1960’s, Bristol Health’'s Counseling Center has provided counseling to individual adults, couples and families who seek assistance for a wide variety of issues. Problems range from temporary difficulties to complex long-term problems that affect emotional and physical health. JOB SUMMARY: Psychiatric Advanced Practice Registered Nurse who provides direct adult health services across the healthcare continuum including health screening, health maintenance, and health promotion. Provides appropriate evaluation and management of health problems by approved protocols established per adjunct medical staff appointment, which includes collaboration and referral to physicians and other health care providers.

Requirements

  • Current CT APRN/RN license
  • ANCC National Certification
  • DEA license
  • Minimum of two years psychiatric care experience.

Responsibilities

  • Provides appropriate quality clinical services to patients and families.
  • Under collaborative relationship with a psychiatrist, responsible for psychiatric assessment, diagnosis, treatment, and follow-up of acute and chronic health care issues.
  • Interprets clinical indicators and integrates assessment materials to make advanced clinical judgments and diagnosis.
  • Develops plans of care and initiates/implements treatment.
  • Evaluates that care by using a systematic approach: through history and physical appraisal, interpretation of laboratory tests, or through evaluation of other available data.
  • Initiates and provides on-going pharmacological management (as outlined in the medical staff appointment/privileging).
  • Indication management includes evaluation, monitoring, education of patients with focus on risk/benefits of medication, drug interactions, complications due to co-existing medical conditions, and potential drug/medication misuse by patients.
  • Orders laboratory and other diagnostic procedures that are commonly performed in the assessment and treatment of psychiatric and substance disordered patient populations.
  • Explains laboratory results to patients and makes referrals as necessary.
  • Responds appropriately to emergency situations.
  • Is available to other staff for intervention/consultation in the case of medical emergencies.
  • Assesses educational needs of patients and families and provides appropriate teaching to meet those needs.
  • Assesses patient and family knowledge and understanding and implements teaching based on needs.
  • Educates patients and/or families about medical issues, prescribed treatments, and/or medications in a manner that is clear and understandable.
  • Works collaboratively with other providers.
  • Works in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration, and referral.
  • Works closely and effectively with other staff members on the multidisciplinary treatment team to ensure coordinated clinical care.
  • Coordinates treatment and flow of clinical information with outside medical care providers to effectively implement the plan of care.
  • Maintains required documentation.
  • Maintains legible, accurate and confidential medical records.
  • Documents all evaluations, procedures, outcomes, education, and referrals consistent with departmental, TJC and state regulatory standards.
  • Documents on-going evaluation of efficacy, side effects and medical sequelae of prescribed medications.
  • Seeks and documents consultation with collaborative physician as needed and outlined in the collaborative agreement, but once a quarter at a minimum.
  • Maintains responsibility for own professional development.
  • Regularly and systematically (but minimally once per quarter) selects appropriate cases for review with the collaborating physician; will review plan of care and patient outcomes.
  • Regularly evaluates own strengths/weaknesses, developing annual professional goals.
  • Maintains/expands professional skills through yearly CEU participation.
  • Analyzes new knowledge gained from conferences, workshops, professional literature and assimilates this into clinical practice.
