POSITION SUMMARY (include key objectives, accomplishments or outcomes for the position): Practitioner is a medical practitioner who is skilled in providing high quality medical evaluation, diagnosis, and treatment to patients in a high-volume outpatient, federally qualified health center setting. This individual manages patients in accordance with standards of care and approved organizational privileges. The incumbent collaborates effectively with other health care providers, social workers, and other professionals in an integrated and evidenced based care team model approach to care. Assess and redirect emergent conditions through available emergency services consistent with organization policy. Provide afterhours consultative call on a shared, rotating basis in conjunction with afterhours triage service, as per organizational protocols.

Requirements

  • The minimum education level required for the position is the successful completion of accredited Physician Assistant training program or successful completion of an accredited Advanced Practice Registered Nurse training program.
  • Holds Louisiana Nursing License without restriction or holds a Louisiana Physician Assistant license without restriction
  • Maintains collaborating practice agreement or a supervising physician agreement as appropriate with the respective Board as appropriate.
  • Has admitting privileges with a hospital directly or a collaborative agreement with a physician who has admitting privileges at a local hospital.
  • Maintains Basic Life Support (BLS) Certification.

Responsibilities

  • Responsible for providing high quality patient care to all CrescentCare patients in an outpatient clinical setting.
  • Responsible for a minimum of 1600 completed patient visits per calendar year, 133 per month (if a full time FTE) and subject to change based on organizational needs.
  • Collaborates with care team members to provide high-quality patient centered care to facilitate high patient satisfaction.
  • For optimal patient care and compliance reasons, the best practice is to close all notes within 48 hours of the visit. All notes must be completed no later than 5 business days of the end of calendar month when the service was provided. (For example, all notes for the month of July must be closed by August 5th)
  • Operates within the scope of practice as outlined by the CrescentCare approved and granted privileges and this job description.
  • Achieves quality metric goals as defined by medical leadership and quality team.
  • Participates in routine case review with supervising or collaborating physicians.
  • Practices at the top of practitioner’s license and collaborate with care team members to provide high quality patient centered care.
  • Uses data and reporting systems to regularly lead or participate in effective and routine pre-visit planning huddles with the care team.
  • Communicates effectively and respectfully with all medical and operational staff concerning patient care issues and functions as a part of a Care Team, using evidence-based practices.
  • Promotes teamwork, good interpersonal relations and communication among staff at the healthcare setting.
  • Actively participates in team meetings.
  • Provides specialty specific consultation services upon request and within specialty scope of practice.
  • Obtains and documents a health history.
  • Performs and documents complete, system-focused, or symptom-specific examination.
  • Assesses the need for and performs additional screening and diagnostic testing, based on initial assessment findings.
  • Orders, executes, and manages appropriate clinical tests. analyzes and interprets diagnostic findings. formulates differential diagnoses by priority.
  • Documents progress notes, plan of care, evaluation and discharge summary appropriately in the electronic health record for medical record keeping, compliance, billing and reporting purposes.
  • Prioritizes accurate data collection for clinical documentation.
  • Administers or prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
  • Analyzes and evaluates patient data and tests or examines findings to diagnose nature.
  • Utilizes evidence-based, approved practice protocols in planning and implementing care.
  • Initiates appropriate referrals and consultations.
  • Provides health education to patient and patient’s personal support system.
  • Obtains proper informed consent as a prerequisite to any procedure requiring informed consent. This information must include the signature of the responsible practitioner.
  • Facilitates the patient’s transition between and within health care settings, such as internal and external referrals and transferring patients based on medical complexity or scope of practice.
  • Maintains clinical competency, as demonstrated in providing and directing patient care.
  • Follows CrescentCare policies and procedures and appropriate standards of care related to patient care.
  • Consults with physicians and other members of the health care team on complicated patient cases as necessary.
  • Transfers complicated patient cases to a physician as appropriate and determined by medical leadership.
  • Provides 6 weeks of notice for planned vacation.
  • Comply with organizational procedures around patient scheduling.
  • Meets routinely with clinical operations staff to review metrics and goals.
  • Completes required continuing education requirements needed to maintain license.
  • Maintains all needed licenses in good standing – Drug Enforcement Administration (DEA) and Controlled Dangerous Substance (CDS) as appropriate.
  • Collaborates effectively with organizational credentialing processes and credentialing staff
  • Maintains compliance with Federal Torts Claim Act policies, applicable Louisiana licensing board, Occupational Safety and Health Administration (OSHA) and other regulatory agencies’ statutes and standards including CrescentCare standards of care and current clinical practice guidelines.
  • Complies with all organizational and regulatory safety standards and infection control.
  • Participates as required in Peer Review process led by the quality department.
  • Actively participates in quality projects and initiatives.
  • Participate in Health Fairs, employee and volunteer health programs, immunization campaigns, and home visits as assigned and appropriate.
  • All other duties as assigned.
