Requirements

  • Master's degree in Nursing [APRN or FNP].
  • Unrestricted Nurse Practitioner license in NY (or ability to obtain).
  • Certified as a Family Nurse Practitioner through the ANCC/AANP.
  • 2+ years of primary care experience at the NP level.
  • Unrestricted CSR and DEA license (or willing to obtain prior to start).

Responsibilities

  • Conduct annual wellness exams and complete applicable screenings.
  • Address quality gaps in care and conduct prospective chart reviews during unscheduled direct patient care hours.
  • Support specific targeted outreach such as homebound members and member facing events.
  • Assist in conducting clinical chart reviews.
  • Work with the Network Team to assist in training and education programs.
  • Complete Annual Health Assessment (AHA) to assess common conditions.
  • Enter assessment and documentation into designated systems.
  • Perform non-invasive ambulatory screening tests and document additional conditions as appropriate.
  • Assess gaps in current medical care using current clinical guidelines.
  • Evaluate urgent and emergent situations and activate the Emergency Medical System as needed.
  • Make recommendations for disease management, screening, diagnostic tests, and health promotion modalities.
  • Assess the need for and recommend durable medical equipment.
  • Provide health maintenance/health promotion and patient education.
  • Administer vaccines as necessary.
