About The Position

The Nurse Practitioner - Annual Wellness Visit (AWV) role at Greater Good Health focuses on providing exceptional care to senior Medicare patients through annual wellness visits. Requirements

• Current California Nurse Practitioner license.
• Nurse Practitioner certification from either the ANCC / AANP.
• 2+ years of clinical experience as a NP in either Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care.
• Home care experience is a plus.
• Working knowledge of chronic care management and various medication options.
• Current DEA licensure / Prescriptive Authority post-hire.
• Active CPR/BLS Certification.
• Current California Driver's License.

Responsibilities

• Conducting annual wellness visits primarily at the patient's residence, supplemented by telehealth.
• Thoroughly documenting medical histories, symptoms, and medication information.
• Administering various tests and screenings.
• Collaborating with the Primary Care team to ensure holistic patient care.
• Ensuring timely completion of patient records, with access to electronic medical records and portals.
• Providing patient education on preventive care and prescribed treatments.
• Maintaining strict confidentiality in line with HIPAA regulations.
