About The Position

The Nurse-Family Partnership (NFP) home visitor performs comprehensive professional community nursing services in a variety of settings. Meets preventive health needs of the population including low-income, uninsured families and individuals, and promotes positive health behavior. Carries out program activities consistent with Mesa County Public Health priorities and fidelity to the NFP model. Operates duties within the scope of practice designated by the Colorado Nurse Practice Act. Requirements

  • Bachelor’s Degree in Nursing (BSN)
  • Active Colorado Registered Nurse license in good standing
  • Current CPR certification
  • A valid Colorado driver’s license with a responsible driving record with access to a personal vehicle that can be used during work hours for nurse transportation
  • Computer and Technology Knowledge and ability to use Microsoft Office applications including, Word, Excel, and PowerPoint.
  • Experience with Google Applications including Gmail and Drive.

Nice To Haves

  • Experience in maternal/ pediatric health, public/ community health, behavioral health, labor/delivery nursing or any other nursing specialty to meet the requirements of the job, preferred
  • Fluency in Spanish and English, preferred

Responsibilities

  • Provide home visits to women and their families to assess physical, emotional, social, and environmental needs as they relate to health and life course development including establishing realistic goals and measurable outcomes for promoting their development.
  • Provide education and instruction to assist women and their families in attaining targeted goals in areas including prenatal and postpartum care, nutrition, parenting, well child care, family planning, and special health problems; evaluates women and families’ progress toward targeted goals; revises plan of care as appropriate.
  • Link women and families with community resources relevant to their specific needs that may include basic needs such as food, shelter, and transportation.
  • Collaborate care plans with other care providers as indicated by the program.
  • Demonstrate time management by flexibly scheduling and rescheduling a day’s activities based on multiple factors, including client need.
  • Record nursing activities promptly and according to program protocols.
  • Consistently maintain a caseload of 25 active clients.
  • Utilize data routinely to understand and improve job performance.
  • Build and maintain relationships with a diverse range of clients over a period of time.
  • Work to understand and address personal reactions to clients, welcome feedback non-defensively, and make good use of the reflective supervision provided.
  • Work collaboratively with coworkers, other departments and leadership to promote program and agency objectives and improve client services.
  • Actively participate in and promote programs within community coalitions, advisory boards, councils and MCPH events.
  • Act as a public health role model and exemplify the principles of social justice, increase cultural competence, and work to close gaps in health outcomes that impact populations disproportionately.
  • Perform community outreach to promote program referrals and keep relevant knowledge of client resources.
  • Participate in program orientation of students and new NFP nurses including mentoring and providing consultation.
  • Demonstrate proficiency in Microsoft Office, Google Suite and electronically generated nursing documentation.
  • Participates in emergency planning and response activities.
  • Participates in agency-wide activities that may include strategic planning, quality improvement, and/or staff development, as appropriate.
  • Completes necessary training through FEMA, including Incident Command Structure (ICS) courses.
  • When a local declaration of emergency or disaster is declared by County leadership, employees may be required to work as part of a National Incident Management System (NIMS).
  • Performs other duties as assigned, including support of public health measures benefiting the community at large.
