About The Position

The Nurse Navigator position at Advocate Health is designed to provide continuity of care by ensuring smooth transitions between care settings. The role involves developing relationships with patients and their multidisciplinary teams to facilitate navigation through subsequent treatment and follow-up, reflecting continuity of care. The Nurse Navigator will complete nursing assessments to identify individualized patient needs and collaborate with multidisciplinary teams to develop interdisciplinary care plans. The position supports patients and families by providing education, addressing psychosocial needs, advocating for patients, and identifying barriers to care throughout the continuum of care. The Nurse Navigator will also provide expert clinical advice, coordinate timely and evidence-based care, evaluate and design patient education materials, and assist leadership in developing program vision and quality initiatives.

Requirements

  • Registered Nurse license issued by the state in which the team member practices.
  • Bachelor's Degree in Nursing.
  • Typically requires 5 years of experience in nursing, including performing nursing assessments and care plan development.
