About The Position

The Navigator functions as a coordinator for patients through an episode of care. The Navigator collaborates with surgeons, primary care providers, rehabilitation specialist, and other health team members to provide high-quality care for surgical patients following established standards and practices. The surgical Navigator guides patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. The delivery of professional nursing care at Stormont-Vail Health is guided by Jean Watson's Theory of Human Caring and the theory of Shared Governance, both of which are congruent with the mission, vision, and values of the organization.

Requirements

  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to read, analyzes, and interprets general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to effectively present information and respond to questions from physicians, patients, co-workers and the general public.
  • Professional nursing theory and practice to give and evaluate patient care: • Medical equipment and instruments to administer patient care • Common safety hazards and precautions to establish a safe work environment
  • Ability to define problems collects data, establish facts, and draw valid conclusions.
  • Ability to interpret an extensive variety of technical instructions in the mathematical diagram form and deal with several abstracts and concrete variables.
  • Applying and modifying the principles, methods and techniques of professional nursing to provide on-going patient care.
  • Developing and maintaining department quality assurances.
  • Establishing and maintaining effective working relationships with patients, medical staff and the public.
  • Registered Nurse - KSBN Required
  • Basic Life Support - BLS Required within 90 days of hire.
  • Additional certification may be required for certain patient care settings or populations. The department director will provide specific information and the timeframe in which such certification must be attained at the time of hire/transfer into department.

Nice To Haves

  • Bachelor's of Science in Nursing (BSN) Preferred
  • 3 years Experience related to department specialty. Preferred
  • 3 years Professional nursing or directly related experience in acute care or outpatient clinic setting. Required
  • 3 years Professional nursing experience in a surgical setting and /or case/care management experience. Preferred

Responsibilities

  • Collects pertinent data and information relative to the patient, situation, or setting.
  • Analyzes assessment data to determine actual or potential problems, and issues.
  • Develops a collaborative plan encompassing strategies to achieve expected outcomes.
  • Identified expected outcomes for a plan individualized to the patient or situation.
  • Promote infection prevention through use of standard precautions, proper procedure in dressing changes, wound care, hand hygiene, and cleanliness of the patient rooms/department.
  • Implements identified plan through coordination of care and employs strategies to promote health and wellness.
  • Evaluates progress toward attainment of goals and outcomes.
  • Delivers professional nursing care guided by Jean Watson's Theory of Human Caring illustrated by creating caring relationships, taking time to hae uninterrupted moments with patients and displaying unconditional acceptance and respect.
  • Promotes a mutually respectful environment that encourages the exchange of ideas and supports the effectiveness of professional relationships and integrates ethics in all aspects of practice.
  • Demonstrates advocacy in all roles and settings.
  • Practices Diversity, Equity, and Inclusion principles in their daily work by respecting others' uniqueness, perspectives, backgrounds, or beliefs.
  • Communicates effectively in all areas of professional practice.
  • Mentors nurses new to their role for the purpose of ensuring successful enculturation, orientation, competence, and emotional support.
  • Supports students to enhance their knowledge, skills, and abilities.
  • Commits to lifelong learning through critical thinking, self-reflection, and inquiry for personal growth and development.
  • Demonstrates willingness to participate and lead nursing practice in process evolution, the scope of the Stormont Vail Health professional practice model.
  • Utilizes appropriate resources to plan, provide, and sustain evidence-based nursing services that are safe, effective, and financially responsible, and used judiciously.
  • Conducting initial patient intakes (collecting patient information, insurance information, referral source, brief relevant clinical information, etc.)
  • Complies with all policies, standards, mandatory training and requirements of Stormont Vail Health
  • Performs other duties as assigned
