Requirements

  • Missouri Board of Nursing RN or Nurse Licensure Compact multi-state RN. When primary state of residency changes, compact state RNs must apply under new state of residency within thirty (30) days.
  • One of the following:
  • --Bachelor of Science in Nursing degree and three (3) years of RN experience; or
  • --five (5) years of RN experience; or
  • --one (1) year of RN experience and five (5) years of LPN care coordination navigator experience.

Responsibilities

  • Coordinate care along the continuum for specified disease processes or populations.
  • Act as a resource for patients, families, nursing staff, and other health care professionals in their area of expertise.
  • Provide education on disease processes and individualized treatment plans to patients and caregivers.
  • Drive best practices around programmatic quality indicators and lead/participate in quality/performance improvement activities.
  • Create processes specific to the disease area of focus to ensure access to care is achieved in a timely manner.
  • Deliver transitional care per area of specialty to ensure an appropriate level of care and compliance with a treatment plan.
  • Provide triage/symptom management and route the patient to the appropriate level of care.
  • Assure care pathways/order sets are followed per disease specialty.
  • Review disease-specific mortalities and 30-day readmission for improvement opportunities.
  • Provide populations at risk with systematic, coordinated care management to reduce fragmentation and cost of care.
  • Provide care management services as contractually obligated.
  • Reduce variations in care and coordinate amongst specialty and primary care providers.
  • May complete unit/department specific duties as outlined in department documents.
