We're hiring a Nurse Navigator at SL Rheumatology Bone Health Clinic in Meridian! What You Can Expect: As a Nurse Navigator RN in the Rheumatology Bone Health Clinic, you will be a key partner in delivering exceptional, patient-centered care. Working side-by-side with licensed providers, you will help evaluate patients and support care delivery within established nursing scope, policies, and procedures. In this dynamic role, you will serve as an extension of the provider team, guiding patients through their bone health and osteoporosis care journey. Your work will focus on improving quality of life, reducing symptoms, preventing fractures, and avoiding unnecessary hospitalizations through proactive care coordination. You will support patients with symptom monitoring, medication management, care transitions, caregiver education, and ongoing clinical follow-up. You will collaborate closely with providers, the program manager, and support staff to assess patient needs, coordinate care plans, and screen new patient referrals for Bone Health services. As a valued member of a multidisciplinary team, you will build meaningful patient relationships and play an essential role in helping patients stay active, informed, and supported throughout their care Schedule: Monday, Tuesday, Friday - 8:00a-4:30p. Wednesday - 8:00a-5:00p, Friday - 8:00a-4:00p.

Requirements

  • Bachelors of Nursing Degree
  • 2 years' of RN experience
  • Current RN License in the State of Practice
  • Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross.

Nice To Haves

  • Previous experience in Ortho, Rheumatology, or Endocrinology experience is a plus
  • Any case management experience
  • Familiar with medically complex patient population
