Job Description I. JOB SUMMARY The Nurse Navigator manages patients to ensure optimal health outcomes and achieve quality measures. Patient’s needs are coordinated with the office to assess knowledge of deficits related to self-care, provide disease management coaching, identify needs that may require referral to community resources, and improve patient quality of health and wellness. Responsible for the Cerner EMR utilization for Nursing staff and Providers. Providers Patient Teaching support. Assists with Research protocols for patients. Develops Commission on Cancer Oncology Nurse Navigator requirements I. ESSENTIAL JOB RESPONSIBILITIES a. Primary responsibility for the Cerner EMR oversight, next steps in development, training and education for Nursing staff and all Providers. b. Supports and assists with all patient education needs. c. Serves as the Research program back up support. d. Meets the Commission on Cancer Nurse Oncology Nurse Navigator requirements. e. Serves as a liaison between the patient, provider and community referral agencies. f. Performs needs-based assessment of clients. Collects specific data, assesses the client’s condition, provides accurate consistent information, formulates and recommends a plan, negotiates appropriate options with the client, and validates the plan with the client for completeness and accuracy. g. Serves as a resource for patients based on the patient’s self-care knowledge deficits or non-compliance with prescribed plan of care. h. Directs patients to the appropriate referral agency based on the needs assessment. Assists in scheduling appointments for client, as needed, with appropriate provider and services and encourages appropriate use of LMH resources and services. i. Acts in the role of client advocate to assist them through the health care system and community resources. j. Reports pertinent observations and reactions regarding clients to the appropriate person (i.e. primary care provider, social work, or Director) k. Maintains a patient centric, customer-driven professional attitude. Demonstrates awareness of own values and attitudes and how they may affect information given to a client. Provides non-judgmental advice based on protocols and resources. l. Collects and updates demographic information, as appropriate and ensures that complete and accurate documentation is recorded in the computerized database for each contact. m. Functions independently and demonstrates leadership in decision making, communication, and problem solving techniques to resolve clients’ questions/concerns. Collaborates with Social Worker, healthcare providers and Director as needed. n. Performs other duties as assigned.

Requirements

  • Current Registered Nurse license to practice in Kansas
  • Current Basic Life Support for Healthcare Providers (BLS)
  • Minimum of five (5) years of RN experience
