About The Position

The Oncology Nurse Navigator plays a pivotal role in delivering comprehensive education and support to patients undergoing chemotherapy and immunotherapy. This position ensures that patients understand their disease processes, treatment plans, and management of side effects. The navigator tailors educational interventions to individual learning needs and collaborates with healthcare teams to promote evidence-based practices, ensuring the highest standards of patient care. The Navigator works closely with the provider care team to develop workflow that ensures best practices are adhered to.

Requirements

• Current Registered Nurse license in Indiana.
• Two – four years of oncology clinical nursing experience.

Responsibilities

• Assist patients in understanding their diagnosis, treatment options, and available resources, including educating eligible patients about clinical trials and technologies.
• Assist in placing patients plan of care and submitting/reviewing QCL to precertification team.
• Act as a liaison between the family and the inter-disciplinary team by educating and assisting with the understanding of the plan of care to patients, families and other members of the healthcare team.
• Provide support to patients with issues or concerns regarding resources needed (i.e. transportation, insurance, support groups etc.).
• Make referrals for financial assistance as appropriate.
• Follow patients throughout their care plan, coordinating with inpatient management and external infusion centers as needed. Coordinate the scheduling and administration of chemotherapy treatments.
• Collaborate with external facilities to ensure prompt treatment initiation, addressing any formulary issues or treatment shortages as they arise.
• Address patient concerns and complaints, escalating issues to management as needed to enhance overall patient satisfaction.
• Coordination of value-based care programs.
